for blood sample collection
Information for
blood sample collection:
1. Please collect 3-5 ml
of blood in a purple topped EDTA-tube
2. Label the tube with the
following information: Dog’s register name, registration number, breed and
date
3. Invert the tube (8-10)
times to prevent clotting
4. Ship to the address at
the bottom of this page. Please let us know to expect your sample by email in
advance (ranja.eklund@helsinki.fi)
5.
Two to three day delivery services are suitable. However, schedule the shipment
so that it will NOT arrive during a weekend or holiday period
6.
Include this form (you can print it out) and a copy of pedigree
All
information regarding the dog´s or owner´s names will be kept confidential.
Results will be released only by the owner´s consent.
